I Agree to Adopt under these terms

Completion of the Voluntary Release Waiver and Indemnity Agreement is required to adopt.

I have completed the Voluntary Release Waiver / Indemnity Agreement. (ATTENTION) This is a requirement/prerequisite to be completed using the link above^

1. I understand there is a period of 6 months of this contract during which time a representative from Find A Home Give a Home (FAHGAH) may inquire and/or inspect this pet and my household to insure proper care is taken as agreed. THE PET MAY NOT BE KEPT OUTDOORS PERMANENTLY. Any other type of animal will be maintained as agreed upon in this contract. I WILL MAINTAIN ANY DOG ON HEARTWORM PREVENTATIVE MEDICATION PER MY VETERINARIAN. My application will become part of this contract and is represented to be truthful.

2. I understand this pet is entrusted to me in assumed good health (except if noted) and I will continue to maintain good veterinary care and abide by the laws governing the area in which I live (county animal control ordinances). I will take the pet to my veterinarian on a regular basis.

3. I agree to hold harmless FAHGAH and more its volunteers, trustees, staff, etc., for any liability or damages should this animal become a problem to myself or others. I understand this animal's background may or may not be as represented by persons placing the animal for adoption with FAHGAH.

4. I understand that UNDER NO CIRCUMSTANCES MAY I GIVE THIS PET AWAY, SELL IT, ABANDON IT, OR TAKE IT TO THE LOCAL POUND. If I am unable to keep it as a companion, I will return it to FAHGAH. If I return a cat, it must have a recent (within three days prior to return) FEL/FIV combo test and a fecal exam. If I return a dog, it must have a recent (within three days prior to return) heartworm test and fecal exam. I ACKNOWLEDGE THIS CONTRACT IS LEGALLY BINDING AND NONTRANSFERABLE.

5. If this pet is permanently removed from the address listed herein, or either lost or killed, I will notify FAHGAH within five days. If lost I shall actively seek to recover it. You agree to make an immediate and serious effort to find the Animal if it becomes lost, by (a) filing lost reports with the local police, animal control authorities, animal shelters, SPCAs and local veterinarians and pet stores, (b) post lost Animal signs (c) place lost Animal ads in your local paper (d) contact us for additional support and advice. Furthermore, Adopter understands that if the loss was the result of negligence or failure to comply with the covenants of this Agreement, FAHGAH Pet Rescue reserves the right not to return the Animal and to place adoptee into a new home

6a. All animals that are up for adoption through FAHGAH have been spayed or neutered unless specified in the application.

6b. If the pet is not currently spayed or neutered, I agree to pay for the procedure and provide proof before the pet reaches 6 months of age or otherwise agreed under additional remarks.

7. I UNDERSTAND THAT IF ANY OF THESE CONDITIONS ARE NOT MET I MUST SURRENDER THIS PET TO FAHGAH UPON DEMAND AND IT SHALL BE MY RESPONSIBILITY TO PROVE CONDITIONS HAVE BEEN MET. I UNDERSTAND I AM RESPONSIBLE FOR LIQUIDATED DAMAGES IN THE AMOUNT OF $1000 IF I BREACH THIS CONTRACT AS WELL AS A COURT COSTS AND ATTORNEY FEES IF REQUIRED. I AGREE TO ALLOW FAHGAH REPRESENTATIVES TO ENTER MY PROPERTY TO REMOVE THE ANIMAL FROM MY CUSTODY IF I DO NOT COMPLY WITH THE TERMS OF THIS CONTRACT. Should FAHGAH be required to take legal action to recover an adopted animal or otherwise enforce the provisions of this contract, THE UNDERSIGNED ADOPTER AGREES that the place of venue shall be Lee County, Florida.

8. If I rent my residence, I will provide a copy of the landlord's agreement for me to maintain this pet in the residence. I have verified with my HOMEOWNER'S INSURANCE company that there is NO policy against certain breeds of dogs.

9. I am of legal age.

10. I agree to pay the adoption fee and microchip fee.

11. You agree that this animal is being adopted as a family pet and will be given extensive house privileges; i.e. sleep inside the house, be allowed freedom of the house when the family is home and will not be permanently restricted to the outside at any time.

12. You agree that the Animal will not be used a guard animal, or for a purpose other than a companion animal, a friend to you and your family.

13. You agree to no alterations to the animal's appearance, such as docking of ears and tails. Regular grooming is required.

14. You agree that this animal is being adopted as is, without evaluation of behavioral issues, (i.e. biting, destruction, escapism, fearfulness, dominance, excessive barking etc) and to follow the recommendations in resolving any issues as an effort to maintain custody of the adopted animal. Any damages (financial, physical or personal) due to behavioral issues is the sole responsibility of the Adopter.

15. You agree that the Animal will be properly restrained while in a moving vehicle, preferably using either a secured crate or seat harness when transported in any vehicle. Adoptee will never be transported in the back of an open vehicle or pick-up truck. Adoptee should not be allowed to ride with his/her head outside the window of a moving vehicle. *

16. You agree that you will not euthanize the Animal except in the case of the Animal’s terminal illness or injury, or old age accompanied by pain and suffering and in that case, the euthanasia must be performed by a licensed veterinarian. *

17. You Agree that adopting an animal is a lifetime commitment. If you decide to not keep the Animal, Adopter hereby agrees to return the Animal to FAHGAH Pet Rescue. Importantly, You agree to give FAHGAH Pet Rescue ten working days to secure a kennel, foster or location for the animal and you will provide safe and loving care of the Animal until it is returned. Adopter agrees to safely transport the Animal to the determined kennel or veterinarian facility and will return any/all paperwork and supplies provided by FAHGAH Pet Rescue at that time

18. You agree that if at any time in the future you cannot continue to provide proper care for the Animal, you will not abandon, give or sell the Animal to another person, company, organization, medical research, pound or animal shelter. You agree to do one or more of the following: A. Obtain approval from FAHGAH Rescue to transfer the ownership of the Animal to a friend or relative. Rescue must approve the new adopter who will be required to sign an agreement similar to this one. B. Agree to cover the costs to board Adoptee for an agreed-upon time, allowing Rescue to actively seek a new home and then relinquish the Animal to Rescue when a home is found. Note: Boarding costs are $75.00 per day. C. Return Adoptee to Rescue.
